To receive your $6 mail-in-rebate:
1) Purchase specially marked Physicians Formula® Bronzers between 4/01/09 and 3/31/10.
2) Print out and mail this completed form along with the MAIL-IN-REBATE label attached to the product and
your original sales receipt with purchase price circled.
3) Rebate must be postmarked by April 15th, 2010.
